We keep a record of the health care services we provide for you. You may ask us to see and copy that record. You may also ask us to correct that record. How we use your information
Treatment: to care for you, and to coordinate with other providers treating you, such as your family doctor or an imaging center.
Payment: to bill you, or an insurer you ask us to bill.
Office operations: scheduling, appointment reminders and running our practice. Our secure practice software and other vendors who handle your information are required by contract to protect it.
Family and friends: we may talk with someone involved in your care if you agree or don't object.
When the law requires or allows it: for example public health and safety reporting, reporting abuse or neglect, licensing board audits, court orders, and workers' compensation.
Only with your written permission: any other use, including sending records to an attorney, marketing, or selling your information (which we never do). You can cancel a permission in writing at any time.

See and get a paper or electronic copy of your record, usually within 30 days, for a reasonable copying fee.
Ask us to correct your record.
Ask us to contact you at a different address or phone number.
